This patient lives out of state and came to me to discuss doing a beard-to-scalp transplant. He had three previous hair transplants elsewhere - 2 FUT and one FUE (about 8000 harvested grafts). As you can see, his donor is significantly thinned out, and also, despite 8000 grafts transplanted into his scalp, the coverage over the top of his head needs more density. Specific areas over the top of his head required more fill-in than other areas. (I marked to indicate areas to fill in with more grafts.) He has a full beard, making for a good donor source, and I can usually harvest 2000 grafts from most beards and up to 3000 grafts from heavy beards. (This can all be done in one day.) We elected to take 2000 grafts from the beard and place them in the areas indicated. I also included Exparel, a liposomal molecule that surrounds the Marcaine, in order for the lipid to slowly dissolve and create a longer-lasting pain relief after the case.

The procedure went well. Our patient flew in the night before and flew home on the evening of the case. After ten months, the patient sent follow-up pictures; as you can see, the areas filled in nicely, and he still has more beard donor if that were to be needed.
